Bringing food allergy management and awareness to your community

School Nurse Survey

Dear School Nurses,

As you know, the roles and responsibilities of school nurses continue to expand and grow as students have increased medical needs and the importance of school health is being recognized.

It is our intention to learn from you what challenges our school nurses are up against when it comes to food allergy and anaphylaxis. The purpose of this survey is for research. The results of this study will provide us with a sense of where current and future resources need to be emphasized and future talking points for the importance of school nurses, school health, staff education, etc. The findings can help state school nurse consultants define unmet needs and current strengths.

Your participation is completely voluntary. You may choose to stop taking the survey at any time. You may also choose to skip any question you do not feel comfortable answering. To protect your identity as a research subject, no identifiable information will be collected. In any publication about this research, no private information will be used.

Your participation in this study will take about 15 minutes. We ask that you take this survey only once. We know how incredibly busy you are, and we thank you in advance for your time and consideration!

If you consent to participate in this study, please click on the button below to begin the survey.

Sincerely,

Julie Wang MD and Michael Pistiner MD, MMSc
